The “Insurance Worksheet” is provided for you to contact your insurance provider and keep for your records. The “Notice of Privacy Practices for Personal Health Information” outlines how medical information may be used and disclosed. Both documents are included in the New Patient Packet or can be downloaded separately by clicking on the link.

Medbrige is a site used by Game Changer Physical Therapy to provide you with access to videos and instructions for the individualized home exercises that were prescribed during therapy sessions. To access your home exercises, click the link on our website and enter the access code that was given to you either in the clinic or via email. Once you enter the code, you will see the exercises that were discussed during your visit with videos and written instructions on how to perform your exercises. Use this feature as often as needed to make sure you are doing your exercises correctly. Remember, quality over quantity. If you don’t do your exercises correctly then they aren’t doing you any good. Pay attention to any notes that have been left in your exercise program. If you experience any pain during your exercises, stop and contact your physical therapist.
